Cómo usar este Nómina Certificada (Formulario WH-347) en US
Complete primero el encabezado: nombre y ubicación del proyecto, número de contrato, la determinación salarial de la que salen las tarifas, el número de nómina (comience en 1 y continúe consecutivamente) y la fecha de fin de semana. Marque si presenta como contratista principal o subcontratista, y si es la nómina final del proyecto.
Luego cada trabajador: nombre, el número identificador —solo los últimos cuatro dígitos del SSN—, la clasificación exacta de la determinación salarial y las horas de domingo a sábado separadas en tiempo normal y horas extra. Ingrese la tarifa por hora, el crédito de beneficios a planes o fondos y los beneficios pagados en efectivo. El bruto del proyecto se calcula solo; el bruto de todo el trabajo se ingresa cuando el trabajador también trabajó en otra obra esa semana. Las deducciones se detallan y el neto es lo que el trabajador recibió.
La página 2 es la Declaración de Cumplimiento y es lo que convierte la nómina en 'certificada'. Las certificaciones 1, 2, 3 y 6 siempre se hacen; la 4 cubre aprendices registrados ante la OA o una SAA; la 5 cubre beneficios pagados a planes aprobados o en efectivo. Firma el contratista, el subcontratista o quien supervisa el pago de salarios (29 C.F.R. § 3.3(b)); la falsificación intencional conlleva enjuiciamiento (18 U.S.C. § 1001; 31 U.S.C. § 3729) e inhabilitación.
Cuando el dinero es estatal o local, se aplica la ley de your state (Tex. Gov't Code § 2258.021); los registros se entregan a solicitud de la agencia.
