如何在美国使用认证工资单（WH-347 表格）
先填写表头：项目名称与地点、合同或项目编号、工资率所依据的工资裁定、工资单编号（本项目从 1 开始连续编号）以及周结束日期。勾选总承包商或分包商；若为该工程的最后一份工资单，请标记为最终工资单。
然后逐一录入工人：姓名、识别号（仅 SSN 后四位）、与工资裁定完全一致的工作分类，以及周日至周六按日分列的正常与加班工时。填写小时工资率、缴入计划或基金的附加福利抵免以及现金支付的附加福利。本项目总额自动计算；只有当该工人本周还在别处工作时才填写全部工作的总额。扣款须逐项列明，实发工资即工人当周实际所得。
第 2 页是合规声明，它使工资单成为“认证”工资单。第 1、2、3、6 项始终勾选；第 4 项适用于在 OA 或 SAA 注册的学徒；第 5 项适用于缴入批准计划或以现金支付的附加福利。签署人必须是承包商、分包商或监督工资支付的管理人员或雇员（29 C.F.R. § 3.3(b)）；故意作出虚假陈述将面临起诉（18 U.S.C. § 1001；31 U.S.C. § 3729）并被取消承包资格。
若资金来自州或地方，则适用您所在的州自己的现行工资法（Tex. Gov't Code § 2258.021）；记录须按机构要求提供。
