Avval sarlavhani to‘ldiring: loyiha nomi va manzili, shartnoma yoki loyiha raqami, stavkalar olingan ish haqi belgilashi, hisobot raqami (ushbu loyiha uchun 1 dan boshlab ketma-ket) va hafta tugash sanasi. Bosh pudratchi yoki subpudratchi ekaningizni belgilang; agar bu loyihaning oxirgi hisoboti bo‘lsa, yakuniy deb belgilang.
So‘ng har bir ishchini kiriting: ismi, identifikatsiya raqami — faqat SSN oxirgi to‘rt raqami, ish haqi belgilashidagi aniq tasnif va yakshanbadan shanbagacha kunlik oddiy hamda qo‘shimcha soatlar. Soatlik stavka, rejaga yoki jamg‘armaga qo‘shimcha nafaqa krediti va naqd to‘langan nafaqalarni kiriting. Loyiha bo‘yicha umumiy summa avtomatik hisoblanadi; barcha ish bo‘yicha summani faqat ishchi o‘sha hafta boshqa joyda ham ishlagan bo‘lsa kiriting.
